Parents/Guardians of students WHO HAVE STILL NOT APPLIED FOR A DEVICE TO SUPPORT LEARNING, BUT WHO ARE IN NEED OF AN IPAD OR COMPUTER TO SUPPORT SUCH LEARNING:
PLEASE complete the survey application below so that the DOE can work on mailing a device to you for your student’s remote learning needs. Please fill out all sections of this survey and hit “submit” when you are done. The DOE will contact you to ensure that your needs are met in order to support your child/ren.
